To thank you for helping us choose the music we play, we want to give you TWO chances to win a great prize.

All you have to do is complete the music test and your name will be entered into the monthly prize draw to win a bundle of ten new Cd albums by some of this years coolest and biggest artists plus you will also be entered into a special prize draw to win this year's must-have gadget, the fantastic iPad Air.

Everyone who successfully completes each music survey will be entered into this draw and if you complete more than one survey, you will be entered on each occasion.

* By submitting this form you are agreeing to take part in a music panel to help choose and shape the music we play. As such you may be contacted by P1 Research and/or UTV Media to arrange your participation. Prize T&Cs:

1. The monthly prize draw for the CD prize pack will be made on the last working day of every month and the winner will be notified by e mail. If you do not respond and claim the prize within 7 days of receiving the e mail, we will make a secondary draw and award the prize to the next person whos name is independently drawn. We cannot guarantee the artists/albums and the albums will be subject to availability. No correspondence will be entered into and the choice of CD's will be at the discretion of UTV Media (GB).
2. The prize draw for the iPad Air. Every successfully completed survey will result in one place in the draw. The winner will be notified by e mail. If the iPad Air has not been claimed within 7 days of receiving the email notification we will make a secondary draw and award the prize to the next person whos name is independently drawn.
